Essential Functions:
Work with doulas, OB/GYN centers, and P3 Center Supervisor and Coordinator to plan for the transportation of clients of the Every Woman Counts doula project; Drive clients in need of transportation to and from appointments and workshops using NFMMC's Care Van; Arrange Medicaid transportation and/or other transportation services for off-hours clients; Call clients to remind them of their scheduled transportation; Establish supportive relationship with up to 132 clients needing transportation over a two-year period; Assess families for immediate needs and provide referrals to appropriate programs; Maintain a log of clients served and services provided; Track statistics for reporting purposes; Assist with other P3 Center needs, as requested by Supervisor and/or Coordinator.
REQUIREMENTS:
Excellent communication skills.
Must have a valid driver's license.
Ability to work flexible hours, including evening and weekend hours.
High School Diploma/GED; Writing ability sufficient to provide adequate documentation and reading ability to the level necessary to comprehend training materials and assist others in filling out forms; Bilingual skills, depending on the community and families being served.
Familiar with the geographic area and the population to be served.
